Mission Medical - Proof of Coverage

I am trying to add my RM daughter to my health insurance, but I need proof of prior coverage from Mission Medical in order for that to happen. Does anyone know how I do that? I haven't been able to find a way to contact Mission Medical directly. Any ideas? I need to get it done soon.
