Reimbursement Form/Payment Request Form

I have watched and read the training on expenditures and it talks about a form to fill out and attach to expenses but I don't know where to find this form. Does the church have a form like they talk about? If so, where can we order them?

But you can find some generic samples here to use or adapt.

As another option, you also may check with your Stake Clerk. I have been in Stakes where the Stake provides a form for each ward that they want the ward to use. Normally this is for ease and consistency during the Stake Financial audits (this same stake also prescribed a single filing plan for all the wards to follow).

Here are ours. The open office version and the pdf version. In the open office version the check box with the check in the lower left corner is there so it can be cut and pasted up without going and finding it. The rest is a simple matter of typing. The .pdf form version is more recent but easier for an inexperienced user to use.

PS we also keep the paper ones in a tray in the office so people can pick them up there too. I have 2 users who fill it in on the computer and print it. The rest are still manually filling the paper in.

Attached are Forms that I have used in both the Ward and Stake Reimbursement process. This is set up to be a 'front and back page' form with the Stake differing a little on the Categories.

Hope it can be of help.

I send the forms to all Stake Auxiliiary presidents and keep copies in the Stake and the Ward clerk's office.

Here is ours for the Stake, we love it. The PDF is fillable. If you wish to use it, you'll need a pdf editor to change the stake info on it. Otherwise, just use the WORD version attached.

We use this form and print it on an envelope. It has worked great for us because they can put the receipts in the envelope and everything is together.

That's a great idea for keeping things organized – I'm going to see about using an envelope form in our stake!
