Patriarchal Blessing submissions

Where does the contact information get pulled from when printing out the Patriarchal blessing submission form for the member to take with them? I thought it came from the lds.org member record but it didn't reflect the changes (or maybe a timing issue) so I had to manually change it on the submission form. Once I updated on the submission form, I could no longer submit it. Is this true that once you change it, the Branch President or Bishop needs to re-submit it?

Are you doing this from MLS or LCR? With MLS, the information about the Patriarch came from the last submission form. I'm not sure about LCR.

I wasn't aware MLS had this information so I have been going through LCR. In LCR, I could not resubmit the form after updating their address.
