I understand there is a 6 or 7 question check list that is available for moving members out of
the ward. After you answered these question and can't find where the member moved,
you can transfer the member to church head quarters. Hope this makes since ..
If this is so, I would like to get a copy....

That list of steps comes up in the process of moving a member record out. It is also available in the Help Center article Move Records Out.

You only see the list if you indicate that the address is not known (where they are moving to).
