Does anyone know if Stakes are assigned a tax ID number? And if so, how do I find out what it is? Stake Conference Report Form

Once completed, where do I mail the Stake Conference Report form? It says one copy goes to the Quorum of the Twelve and one to the Area President. Do I mail both copies to one of those addresses, or mail them separately? If I mail anything to the Quorum of the Twelve, what is the mailing address?

Dissolving a branch or ward

Is anyone aware of a form that must be completed in order to dissolve a Branch or Ward? I cannot find one on the website or any mention of one in the Handbooks. Any help on this would be appreciated.

Sounds like it may just be more an issue of me wanting CDOL to do something that it's not set up for. I'll look into the CDOL setup a little more to see if I can figure out anything else that might help. Thanks for the heads up on the seminary callings being changed--that did throw me for a loop.

The callings have not made it to CDOL. If callings are changed in LCR now, is there a reason those changes are not reflected in the "Directory" portion of lds.org? Is there plans to change that? As much as I can tell, there is not even a position listed on CDOL for Seminary instructors or ...
