As far as I know, each Ward makes their own. Yours has most of the same information that we use, just organized a little differently. I think you're right there. I know the form that we made was completely different from the one attached there. Mostly the same info, just a totally different layout.

It's been a while since I checked, but I believe our stake leaves the lessons out as well. Our ward puts up lesson schedules for the RS though, and the EQ typically goes ahead and just follows the same plan.
