We struggled to convince our FM guys to get rid of junk in our storage shed (we were told we could just dispose of it ourselves if we desired), so at least any general push hadn't made it to our neck of the woods when we went through this recently... ;)

I'd also ensure that whatever led to this occurring, is properly resolved. Both the Stake President and Bishop should be aware of this policy (must be 18 prior to Melch Priesthood and Temple), but perhaps they didn't know this was a fixed rule. No need to contact anyone. To me intent of this portion of the FAQ is to lift this exact burden from clerks/secretaries. I get this every time I try to submit a food order.
Have you successfully used the online order form before, or is this the first time?

Also, did you try using a different computer with your church account to try and create the order? Just looking for more info to maybe help you resolve this.
