On the screen where it says name of New Ward, you need to type in the name of the YSA Ward. If you aren't sure of the name, you can type the address in http://www.lds.org/maps and it will tell you the name of the attached YSA ward. For "ecclesiastical" purposes, you can call the administration office and they will let you know in which unit the records currently reside (available only to Bishops and Clerks). Generally they prefer it if you have a birth date too (can be found on an old ward directory).

The work around we have been doing on this right now is that when a new membership record is received in the ward, we automatically write on it the prior unit information from the Membership Record Update Report. Not as great as having it already on the record, but we are working with what we have.

I know CHQ used to ask the Stake Clerks to do it, but I think they automatically do it now once a member enters the field. Someone is free to correct me on this.

I've been a clerk for the last 3 years and I'm noticing a new change when membership records are moved into our ward. They will give this information to Ward/Stake clerks. Usually having a birthday or the MRN helps them a lot in the process.

On the limited-use book, that would be about right. The book itself has directions stating that it should be kept until all the recommends issued from it have expired. Since limited-use recommends are good for a year, it should be destroyed a year after the last recommend was issued.
