In our YSA ward one of the members of the bishopric's wife can't see the members of our YSA ward in the ward directory on her LDS tools app. Is there a way to grant her access to the ward directory since she and her husband are out-of-unit members?

It might be a difference in Android and iOS.

It's possible she may need a calling.

You mentioned they are out-of-unit members. Does that mean you added them as out-of-unit members to your ward in LCR? I haven't experimented with what that grants in terms of ward directory access and we don't have any out-of-unit members in our ward right now. If that doesn't grant access, then russellhltn is right, they might need callings.

I don't know if the new ward directory & map website has a unit selector the way some other tools do. If so, they might just need to pick which unit's directory they want to see.

The first counselor's wife has not been added as an out-of-unit member. Go to Membership > Manage Out-of-Unit Members and look at the list there.
