legal marriage record

I need a legal record of my marriage to prove my name change for my drivers license in Utah as I moved here from California last year. The problem is that apparently my husband did not register our marriage. In order to declare a marriage (which is apparently what you do when you didn't register a marriage) the signatures of both husband and wife are required. Unfortunately after 15 years we divorced (I kept my married last name) and he has since passed away. We applied for marriage in Ventura County Ca. and were married in the Los Angeles Temple. The certificate the temple gave us is gone and all I have is my divorce decree, my receipt for application for marriage in California and the date of marriage in my church records but my maiden name is not shown on any of these documents. What do I do now? I cannot get a Utah drivers License without a legal document proving my name change.
