If someone's membership has been revoked through excommunication can their records and membership record number be sent back to their ward ?

No. If the ward or stake needs that information, perhaps to reconvene a disciplinary council to consider the return of the member, they will request the minutes of the original council and those minutes will have the information they need.

So if the member was able to return they wouldn't need to baptized again ?

If they were excommunicated, they would need to be readmitted by baptism. If they were disfellowshiped, they would not need to be baptized again.

Your bishop will better be able to answer any of your questions about church discipline, as each situation is different.

Once the re-baptism is done and necessary paperwork is submitted then the record will be sent back to the Ward with the original member number and original baptism date shown.
If the person was temple endowed and/or MP holder then the record will show a note about restoration of ordinances needed.
Once the restoration is completed the record will be as before as if nothing ever happened.

If restoration of blessings is needed (the member held the Melchizedek Priesthood or was endowed), the NEW baptism and confirmation dates are shown until restoration of blessings is completed.
