Is there a sign in sheet for FHC?

We lost our original sign in sheet for our FHC. Does anyone know where I can find another copy?

Something like that would be locally designed and produced. If you are not the FHC Director then cantact that person. If you are the director then contact your stake high counselor over the center for direction.
