Custom MLS Forms

This is different from custom reports.
My first thought about this was on Sunday when our Stake Patriarch (which has been in that calling about a year) which I have helped with computer problems before told me that the form letter they used got messed up and needed help getting it re-aligned to print on the form for the blessing.
I then was asked to make an form that could be easily filled out and sent to the stake in electronic format for recommendations for stake callings.
So the idea is to be able to have custom forms that could be created by the Ward or most likely the Stake that can be filled out in MLS (or in the new version of the website) and then sent back into the Stake.
