Since this is a public forum, I changed the name as it wasn't clear that it was fake. Assuming you are the clerk, you'll need to check the various names in the membership record. In particular, I'd check the Preferred and Household name. Given that in the original, the last name sounded like a first, and the first name sounded like a last, It might have been entered incorrectly.

Please restore [-- Member names removed --] from the last name mistake of [-- Member name removed --] to their correct name "[-- Member name removed --]"

This forum is not a method of contacting the Global Service Center and the Global Service Center is not going to make changes to membership records based on instructions from an elders quorum president. Please talk with your bishop or ward clerk to have this taken care of.
