Annual Charitable Cash Contributions Official Tax Summary

Hi,
A few things:

1. For members' end of year tax summaries, we should be distributing the form described at the page in clerk resources called "Annual Charitable Cash Contributions Official Tax Summary Statement," correct? (find it at: https://www.lds.org/callings/melchizede ... t?lang=eng).
2. It seems like a number of people do not know about this "new form" and have been asking for the old form (called "Annual Charitable Cash Contributions") that showed only the donations in the ward that they are currently a member of. When they come asking for their tax summary from their previous ward, should we just print off the "new" form or send them back to their ward clerk to have him print it?
3. Does the bishop need to sign the "new" form? He did on the "old" one, but I assume that the Treasury Division Representative's signature replaces the Bishop's signature on the "new" form, correct?

That form is for members donating to more than one unit during the year. Check MLS in Finance | Finance Statements |Official Tax Summary Statement for the forms. All other members of the ward should get the tax valid statement, "Official Tax Statement ..." printed from Finance | Tithing Settlement | Donor Statements.

The form a member gets is determined by whether he/she has made contributions to another unit or not. Some members contributing to more than one unit may get the Annual Charitable Cash Contributions Official Tax Summary Statement as well as the local Official Tax Statement to ensure all donations contributed are included. But members only contributing to their current ward will only get the local Official Tax Statement.

Members need only go to their ward clerk or ward finance clerk for the applicable form. They do not need to contact their previous ward.

No. The "Annual Charitable Cash Contributions Official Tax Summary Statement" is sent from headquarters and is a PDF file. All that needs to be done is to print it and distribute it. The form has already been signed by the Treasury Services Division Representative.

Thank you very much for your help! I should have mentioned that I am in a YSA ward in which the majority of members did live in multiple units during 2013, and have had previous members of the ward come asking for their tax summaries for donations that they made while in my ward. This was the reason for question #2.

Given your responses, it sounds like I should check which members were in the ward for all of 2013 and give them the local tax summary, correct? (I printed all of the "new" form that I referenced already.)

For those who live in countries that allow charitable contributions to be deducted from their income for tax purposes, this document provides the information to do so. You should consult a competent accountant or tax adviser for your specific situation.
