Melchizedek Priesthood Ordination Form

Hello- Where can I find an electronic copy of the Melchizedek Priesthood Ordination Form? Is it on lds.org or MLS? Is this currently available to submit online by the bishop? Any input will be appreciated. Thanks Erik

I know it exists in MLS under Forms, as do all other standard forms used for ordinances. I can't say anything about LCR.

Thanks- I was able to take a form on PDF and convert it to word. It can be edited that way. Just looking for an easier way to to things now that MLS is going away. If anyone knows when/where these forms are located on LCR, it would be greatly appreciated.

MLS isn't going away any time soon. All of the functions (including finance) have to be migrated before that happens.

MLS isn't gone yet. There are still a number of things that can only be done in MLS. Anything related to ordinances, ordinations, new records, deaths, marriages and blessings are still only done in MLS. Probably the next major release of LCR will have many if not all of these things but that will probably not be for several months still (if not longer--no specific timeline has been announced).

Go to Membership > Ordinances > Forms on the menu in MLS and then select Melchizedek Priesthood Ordination Record from the list. This form is not available in LCR yet.

Leader and Clerk Resources (LCR) > Membership > Ordinances. Then select Priesthood Ordinance in the "Select an Ordinance" drop-down list box.
