Cannot complete registration

I am trying to set up an account for my husband. I filled in all the blanks, verified his email, then tried to verify his phone. It denied the code. I tried to resend it, but it denied it twice more. Then I was locked out. I waited for a time then tried to set it up again. It tells me that the MRN is already in use. So, I asked it to resend the phone code and it tells me that there is not a phone associated with the account and log in and enter it. I tried to log in with his information and it tells me that the account does not exist.

What am I to do?

Try to recover the username based on his MRN. There may be another account that had been setup in the past.
