Child Record Form

Child Record Form

For members under 18 who require the Child Record Form for consent, how do I generate this? It gave me the option when I was creating the record, but the family was not at church that day so I created the record but waited until today to print the form out. Now all I see if the option to upload consent. I can't see where to generate the form. Thanks!

You can print a blank form by going to Membership > Create Record in LCR.

I have the same question and still not sure how to work this. I can easily create a blank form, have the parent fill it out, but then how do I record this as a permission from the parent?

As far as I remember, when creating the record you have the option to upload the completed form. Similar to the process used when uploading expense receipts.

You have to upload the completed and signed form... You can't continue until the form (or something) is uploaded. If you have a scanner attached to the computer then easy to do that. If not what I do is take a photo with my phone then email the photo to myself. Download the email attachment to the computer and upload that.

I have a follow up question..... I have created many records and have never had to get parental consent, so is this new?

Yes. It's only been effect for a few months.

The requirement has been there for a long time, it is only in the past few months that the church made software changes to enforce a documented "permission" slip (as I understand).

Thanks for the clarification. I meant that the requirement to upload the form is recent but I was a bit too brief. You are absolutely correct that the Child Record form was supposed to be filled out and then signed by the parents for a long time before that--certainly ever since I was first a membership clerk over a decade ago.

Thanks for the replies and info about the forms. Because the parents were not there at the time of making the record I canceled when it asked for permission and it still allowed the record to be made, and the record shows up correctly. I'll just have to go back and get the permission slips signed.
