First post here, although I have been learning much by reading on here once I discovered this forum
My Bishop is asking me to come up with a form to hand out to any new members that move into our Ward to help with pulling their records from the old Ward. He says in other Wards he was a part of (he is retired Air Force so has been all over the world, including being a Bishop at least 4 times now) that there was forms available. He gave me the impression that it was a form that the clerk of that particular Ward did on his own, meaning it was not something Church supplied. He said it was a blank form that the new family/member would fill out with name, current contact info, even went as far as what were past callings, interest's and such. A kind of "get to know me" form.
Anyway, I was hoping someone on here may already be using some sort of form like this and be willing to share it with me. I would be ever so grateful.

Here's a form we used in my YSA ward 6 or 7 years ago – it would need to be adapted, but might be helpful.
