Are physician & dentist documents required attachments for missionary applications?

Are physician & dentist documents required attachments for missionary applications?

As Stake Clerk, I normally assure that the physician and dentist evaluations are attached and fully transcribed in the missionary online recommendation system. I have considered both the attachment and the transcription as mandatory.

I just received an application that was started, but not completed, Out of Unit. The information appears to be fully transcribed and complete, but there are no physician or dentist documents attached. Further, in the section under Photo and Other documents, the Physician and Dentist evaluations are in a category called "Optional Documents."

So, do I need the family to send these evaluations so I can attach them? Or are they truly optional and I should trust that the other Clerk transcribed them correctly?

The last time I did it, which was along time ago, the stake entered the medical information into the system and then kept the paperwork on file until the missionary returned from his/her mission. The process may have changed, but that is the way we used to do it.
