stake/district youth conference registration form

Where can I find online the form:
"2013 Stake/District Youth Conference Registration Form"
I tried so many ways, but nothing. It would be nice if you could find all the forms in one page. Maybe it is there, but I have no idea.

Where did you hear of such a form? I've never heard of such a form existing at a general level. Although the name includes "Stake/District ... form", which sounds like a general form, in my experience such forms are always created at the stake/district level.

There is a medical release form, and an overall Activity Plan that might be helpful, but I don't know of any registration form.

Like aebrown, I've never heard of a general form like that. They're almost always made by the people who are putting on the Conference as they have specific needs for information depending on the details of the activity.
