Where would a Bishop find the electronic forum to fill out the "Ecclesiastical Endorsement" for a seminary graduate?
FYI...if it is possible to get a quick response for the seminary graduation will be Sunday, June 7, 20015. Thank you.

Is the endorsement for seminary, or for the school (BYU) they're applying to?

The endorsement is for The San Francisco West Stake in California which will take place this Sunday, June 7, 2015.

The ecclesiastical endorsement that a seminary student requires to receive a certificate of completion or a graduation diploma is not electronic. All students applying to a Church Education School which includes, Brigham Young University, Brigham Young University-Hawaii, Brigham Young University-Idaho, LDS Business College, and Salt Lake Center are required to have an Ecclesiastical Endorsement.
You may have a bishop, branch president or mission president of the Church of Jesus Christ of Latter-day Saints fill out this endorsement online for you or you can have your Ecclesiastical Leader fill out a paper form. LDS Leaders can be identified by name or by location. Applicants will be required to obtain a 2nd interview with a Stake Presidency or CES Chaplain.
Applicants to the following programs at BYU must also have two interviews to complete the Endorsement.
•Bachelor of General Studies
•English Language Center
•Evening School
•Graduate Studies
•Jerusalem Center
•Law School
•Study Abroad
Students who already have an endorsement for the current Academic Year are considered "Continuing" and will only need to get one interview.
Students "Returning" after a period of more than one major semester, will need to get two interviews.
All students and applicants need a NetId to login. Make sure that you use the same NetId for your Endorsement and application.

For Seminary, there is no form. When the priesthood leader signs the Certificate of Completion or the Graduation Diploma, he is providing an ecclesiastical endorsement. I am not certain who signs the new Certificate of Completion. Beginning this year, each student receives a Certificate of Completion each year, and these students are recognized at Seminary graduation. Again, the signature on the Certificate represents the ecclesiastical endorsement. If the bishop will sign the form but withholds endorsement at the present time, he holds the certificate until the student is qualified, then presents it at a later date.
The Ecclesiastical Endorsement for attendance at a Church college or university is provided online as others have indicated.
