Missionary Online Recommendation Form - Help!

When I try to submit the Privacy Agreements the portal tells me there are two errors, but won't tell me where or what they are. I can't see anything wrong from my end and my bishop can't see any flaws either.

The secretary of our mission asking us for COMMITMENT FORM signed by the Missionary Applicant's parent. where can I get that form? I already see the forms in Print Blank Forms, there is no form there where the parent can sign.

Seems a strange request. Having just quickly skimmed through the Blank Recommendations Form, under Fathers and Mothers information on page 4, there is a tick box stating "Check here if you do NOT want your father/mother to be contacted at all".

Therefore how can a parent sign a COMMITMENT FORM, which incidentally I haven't found either, if the prospective missionary doesn't want them to be contacted?
