90's building audio volume control issues

We have a off and on problem with the chapel audio where the audio is hot (ringing/verge of feedback) however the pulpit audio control panel can not turn it down. It has been reported multiple times over the last several years. So I am not sure what the FM group or sound contractor is doing to resolve the problem each time, but I am guessing they are just power cycling the equipment in the locked sound cabinet.
No feedback is ever given on FIR reports.

When the sound system turns on, the green leds on the pulpit control are at 3 (middle) yet it acts like it is on 5 or higher and no way to turn it down.
There are no other mics plugged in except the sacrament table mic and it is off.

While I have not seen a systems diagram for this building, I am guessing the pulpit volume control panel just sends a control signal to a control device in the sound cabinet. I am guessing that control board is flakey?

IF anyone has experienced something similar in a chapel, or has ideas let me know. Bishops are getting frustrated. 4 wards meet in the chapel each Sunday.

Get the stake president to push the FM group for a lasting fix.
