Subsitute Receipt Form

When there is no receipt for a member reimbursement, what exactly needs to be included in the substitute receipt documentation?

Does anyone have a substitute receipt form they can share?

If a receipt or invoice is not available, the member should provide a full written explanation of the expense, including the date, amount of the purchase, and category. ... bursements
