Does anyone have the on-line navigation to get to the Child Record form ?

In LCR [Membership] --> [Create Record]

If the child already has a record then click on the Ordinance link.
If not click on one of the 3 blank forms at the bottom.

Is the "Child Record Form" the same as the "Consent Form" that must be loaded into LCR?

Yes. The Consent Form is a generic name used to refer to the forms for minors that requires a parent or legal guardian signature on the form. It currently includes the Child Record Form, the Baptism and Confirmation Form (Member of Record), and the Baptism and Confirmation Form (Convert).

Posting your permission in a public forum is not going to work. Please get a form from the clerk or membership clerk of your ward or branch and send back the signed form or a scan of it.

I thought i would let you know that if you are creating a record for a child born in the covenant, you need to print the consent form before proceeding to make the record. The system will allow you to complete the record, but at this point, you will need to print a blank consent form, fill it out completely, have a parent sign it, then upload and attach to the record. I am not finding another way to print a consent form after the record has been created and the blessing has been notated. It would be nice to be able to create the record, fill in the blessing information and then print the consent form to upload.

Consent before ordinance. General Handbook 18.6.3 states: "Before a child is blessed, a clerk uses Leader and Clerk Resources (LCR) to prepare a Child Record Form."

Reversing the order creates problems. The most common is they may take off and you're unable to get the needed information. (Such as when the child is blessed in the grandparents' ward.) But it can also create more serious problems if the parent refuses to sign.

We've had another post here from a clerk who is has a problem because they can't record a baptism because the parent refuses to sign after reading some of the conditions.

So, consent before ordinance.

Do I (can I) create a Child Record with only one parent information and signature on the form!

BACKGROUND INFORMATION
I have a Child Record Form that was filled out, signed by ONE parent (the mom), the blessing occurred, and then I was given the Child Record Form to record the blessing. The only problem is there is no information about the father - the mom says the father has nothing to do with her or the child (no contact, no support). The form was signed by the mom but not the dad. The Child Record form seems to only require one parent’s signature (“Signature of parent or legal guardian “)

I'd contact support. They probably have something for situations where the father is unknown.
