What options are out there for leaders to more easily survey their stake/ward/branch members?

The Church doesn't provide any tools that do what you are asking. Before undertaking this, you may want to have your leaders review General Handbook, 38.8.36.

I don't think the church provides any options for that. You'd have to use a service.
I would caution against any usage where the leadership provides information about the member (such as their emails) to the service, but rather, the leader provides a link to the survey to the members and it's the member's choice about what they share. Members should be free to opt out of using the service.
