Patriarchal Blessing Request Form

I recently had a member come ask me about getting a replacement for his lost copy. I had also stumbled on this in the tools menu, so I sent him the link. He had to create a lds.org account and then go to the above link. Once he logged in, his request was sent. Two weeks later, a copy of his blessing arrived in the mail. He was ecstatic when he popped in to my office on Sunday.
Pretty sweet capability that will bless a lot of people.

Simply follow the instructions on LDS.org (at https://apps.lds.org/pbrequest/) to request your patriarchal blessing. Once it is available, you can download it to your phone.helenjeanbauer wrote:I would like a copy of my PB on my I-phone

My mom's name is still on the church records although she has been inactive for over 25 years. She recently requested a copy of her partriarchal blessing. She thinks she remembers the ward she was in but not 100 percent sure. how can i help her get a copy?

With her full name and birthdate, the ward clerk where she's currently living can request her membership records. Once the membership records are in the correct ward, the clerk can give her her membership record number, which she can use to set up an LDS Account. Using that LDS Account, she can go to "Patriarchal Blessing" in the My Account and Ward menu at the top of LDS.org, where she can request a copy of her blessing.
