Blank Bishop's Food Order form

Blank Bishop's Food Order form

Does anyone know or have a blank Bishop's Food Order form that we can give out to the member's ahead of time to fill out before entering it online? The one's I see online have watermark examples and I do not have access to see which form is the current one being used. The workaround I can use is to have the RS President open the form on LCR, enter a "name" and then do a screen print of it with the name removed. Any help with getting a blank Bishop's order template would be great.

Since the member does not fill out the form, I doubt it will be easy to find one. Normally, the Relief Society president works up a food order with the family under the bishop's guidance and direction, and then the bishop approves it when it is completed and entered by the RS president.

We're trying to handout the blank order form beforehand so that it saves time from sitting with the member in front of a computer to go over each item one by one. I can understand if it is the member's first time and needs to work with the RS President but some of these are recurring orders that if they can fill out ahead of time, the RS President can submit it for the Bishop's approval.

I asked my Relief Society president to visit the member, determine their needs, and then propose a list of items, which I almost always approved. And while she was familiar with the availability of items from the Bishop's Storehouse, she never sat down with a storehouse inventory and let the member use it as a list from which to choose items.

Although some families required more than one food order, we never treated them as recurring orders. We went through the process each time, checking on the actual needs of the family, and reminding them of the sacred nature of resources that faithful members had provided to help them through temporarily challenging times. This also provided an opportunity to review the progress of the family in becoming self-sufficient again.

I am all for efficiency, and the time of a Relief Society president is just as valuable as the time of a bishop. It's just that we felt it was worth the time to continue to teach the family their own responsibility for self-reliance.

This would make an interesting discussion item at your next stake Bishop's Welfare Council Meeting.

If you choose "not on this list" for the member name, then fill out the other required spaces with any character such as "x", you can print the blank form. I find it helpful to take the printed form with me to the home visit as it's easier to see than a screen.
