Just to be clear, Member Tools requires a Church Account with a membership record attached. To log into that account so you'll have to supply name/password.

Tools will also require a PIN. (A numeric-only "password") You set that when you install. If you've forgotten it, you'll need to uninstall the app and wipe any data (at least for Android). Rebooting the phone wouldn't hurt. Then reinstall the app.
Have you searched the Help Center? Try doing a Google search and adding "site:churchofjesuschrist.org/help" to the search criteria.
