Officers Sustained Form printed from MLS is wrong - how to fix?

When we print the Officers Sustained form from MLS it has wrong information (such as: it is showing a Beehive Advisor as the Beehive Secretary).

We've looked at the organizational information to verify that it is correct on-screen (it is), but when we print the form it still shows up wrong.

How can we fix this? Should we do a "Request Unit Refresh Data"?
