When leaders request reimbursement, we have them fill this out and attach their receipt to the back.
Or we usually fill it out for fast offering dispersement.
After the check is written, we attach the check stub to the front so everything is filed together. It also helps us ensure everything was authorized.
But, did we overlook an official form for this?
Is there a better way?
You may need to adjust the budget categories to match your unit.
It was created in open office. The Word .doc may need some tweaking.

As far as I know, each Ward makes their own. Yours has most of the same information that we use, just organized a little differently.

I think you're right there. I know the form that we made was completely different from the one attached there. expence request form
In the MLS training for Church expenditures it says that each ward makes a form that fits their wards needs.
We used the one the training video, but use what ever form the bishop is comfortable with. We also found that when providing a Fast Offering check it was a good idea to have the recipient sign a receipt with the amount of the check on it. In the case of rent assistance we would have a letter requesting a receipt and a stamped bishops envelope for them to return the receipt to us. It helps verify the expense for the auditor.

When I designed the form for our ward, I purposely put enough space at the top so the the check stub could be stapled to the front of the form. That way when the stake clerk comes by for the audit, the whole form is easier to verify at a glance.

Examples of requests used in a stake
Here are copies of the forms that we use for stake officers. We felt it was an opportunity to remind people of the principles and guidelines of using the local unit budget allowance. We have two forms 1) for advances 2) for reimbursements

Redesigned Check Request Form
I was dissatisfied with the layout of the previous form I had designed, and so I changed the layout of the the form hoping it to making it look more like filling out a check.
