Problem creating new missionary application

I cannot figure out how to do a new post, so I am posting here. I am trying to initiate new recommendation for a missionary. I input the record number and birthdate in then hit next. I get an error message "null". ThenI get a box that says "validating" and inside the box says loading... Nothing else happens. Can someone give me some suggestions, please.
