MeetTheMormons.com: Error "Requesting the Film"

There seems to be an error with the "Request the Film" form on MeetTheMormons.com

Many users are getting the following response when they try to submit a request:
"Your submission could not be sent at this time. Please check all fields and try again."

Unfortunately, most of the users who are hitting this issue and using the Disqus commenting system that is only visible if you are on the Error page: http://meetthemormons.com/error

I was hitting this issue and after a bit of debugging realized some of the content on the page was being blocked by my Tracking Protection list in IE (which I have enabled to block ads). I imagine others with similar AdBlock plugins are also hitting the issue.

Just thought I would raise the issue here in case those maintaining the site are not aware of it.
