Help with how to download expense request forms

Please help me with how to download the expense request form. I don't know if that can be possible at the LCR

There is no one expense form for the entire church. Each unit is instructed to develop their own (although their is a sample form on LCR). You'll need to contact your ward or finance clerk and find out how to get a copy of the form your unit uses.

If your unit does just use the sample form then it is available on the Reports menu in LCR Finance but only a limited number of people have access to LCR Finance. It's possible that organization presidencies may have access since they can access the LCR Finance Budget page but I'm not sure (the LCR Access table doesn't mention it).

Is it still the case that wards are encouraged to have an expense request form, now that reimbursement requests are entered directly online by organization leaders? (I don't have access to LCR Finance to see, but I thought that those went away.)

If an expense is entered online then there isn't a need for an expense form. But if requests are submitted on paper then it is still needed. And only the major organizations (EQ, RS, YW & SS) can submit online. Others like public affairs and STS (at the stake level) don't have access. Even clerks and bishopric/state presidency members can't submit requests online. Technically they can enter expenses directly but at least in the US if you enter an expense you are the first approver so it's a bad idea to enter an expense for your own reimbursement.

On the other hand, I accept email reimbursement requests and as long as the email includes the information I need (payee, amount, purpose, category and documentation). I print the email to PDF and consider that the 'expense form.' The main thing is to get the necessary information.

I just created two forms, one for "Reimbursement Authorization" and one for "Fast Offering Assistance Authorization" and submitted them to the Bishop and Stake President for use approval. Those forms have worked for years now during every audit I've been involved in.

Here are some examples (please feel free to suggest improvements)

I don't think there's any reimbursement forms from the church. Each unit comes up with their own.

Since I have not been in a stake clerk calling for over a decade I do not know if the form you are referencing is still used or even available in electronic form. You will need to contact your stake clerk for the form if it still exists.
