Contact your ward clerk to get a copy of your individual ordinance summary (IOS). It contains essentially all the information in your membership record.
If you want to make changes to your record your ward clerk will have to do that for you. Any of your contact information you can change yourself in the online Ward Directory and Map or in the Member Tools app.
