That's a great question. From my perspective (I'm not a temple worker so no inside info here), there's two aspects to the question:
1) Is there a church-wide policy regarding cellphones in the temple? If so, how do I as a regular member, know about the details?
2) If not, are temple presidents given stewardship to decide this temple-by-temple? If so, how do I as a regular member, know about the details?

Implementing a printed version of your "SmartBaptism" plan is a great idea. It's really cool that you've taken initiative to try and solve some of the logistical issues, and also really good that you've reached out for input regarding practicality (if the app is awesome but can't be used in the temple).

PS: many of us have had situations where individual temple workers have approached us with their own personal interpretations of policy -- so even if it's "allowed", you may find workers who feel cellphone usage is absolutely inappropriate in the baptistry. For example, I once had a temple worker advise me to not check my cellphone in the lobby outside of the recommend desk. My friend was checking his smartwatch at the time, and the worker was completely unaware that he was doing the exact same thing as I was!
