Is there a new member information form
I'm in need of a form for a upcoming new member.

You need to carify what you're looking for.
If you're talking about baptizing a 8-yo/member of record, then that would be in LCR.
If you're talking about baptizing someone 9 or older, I'm pretty sure that would be the missionaries.
If you're talking about a form for someone moving into the ward, those have always been created by each local ward. There is no universal form.
