Wrong name in mission call

My son just got his mission call , problem is in church records he is using my last name ,but in his paper works he is using his mother's last name , how can we inform or who do we need to inform about this problem

I suggest you contact your stake clerk. He can investigate and contact global support if needed to resolve the issue.
