Child Blessing Form

I can’t find the form that the parent needs to fill out before the child blessing and naming.

Have you contacted your stake clerk? He is responsible for training you.

The form is found at LCR > Membership > Create Record. At the bottom of the page is the link 'Blank Create Record Form (seven or younger)'. Click it for a blank form. The form can be partially filled out by completing the fields on the page, if you have that information.
