Question about consent forms

Question about consent forms

We’ve had a couple of convert baptisms of children (ages 10 and 14) where the missionaries taught and baptized them. While completing their membership records, submitted by the missionaries, I noticed that the consent form files the missionaries submitted is just a copy of the parents’ signature and nothing else. No verbiage saying that they consent to the baptism or anything. Just their signature. Is that how it’s supposed to be?

Yes. When the missionaries use their system to create a record the e-form is filled out and the parent/guardian signs the filled form on the missionary smartphone.

Which is great when they get all the information correct, but that rarely happens. I then spend a lot of time getting the correct information.

Make sure you talk with the missionaries about missing information. We have had convert baptisms that had all the correct information filled out on the missionary end, but when it came to approving the new record in LCR some of the information was missing - such as who performed the baptism, who performed the confirmation, the place of birth, the birth date. What data goes missing varies from record to record. When this happens get the information from the missionaries as soon as possible and contact the GSD help line to have them look in to it. After calling the GSD and they see the issue I then fill in the missing information and complete the record. I had to hold off entering the missing information because the first time I called the GSD support person did not believe the missionaries entered the information.
