Member move-in/out forms

Being a new membership clerk, I was wondering if there is an existing form to help obtain new member information and gathering information on move-outs as well.

The wiki article New move-ins has helpful suggestions and several forms that other clerks have created. As for moving out records, see the article Moving membership records.
Questions that can benefit the larger community should be asked in a public forum, not a private message.
