GSD = Global Services Department (used to be called the Global Service Center).

I would use the "Support for Local Unit Leaders" section.
I think there is also a special request option in LCR that will let you send the information to them (I haven't had access to LCR for a few years so can't verify): Full member name, membership number, death date and death place should do it.

You can use the Help > Contact Us menu option in LCR to submit a support issue where you can make this kind of request. You could try calling them back into your ward using the meeting house address.

And the GSD told me a few years back that in order for them to record a death they needed an obituary or death notice uploaded to them, I suppose to prevent someone pranking them that a person is dead. So there's that . . .
